Inside Pitanguy Island: Brazil's Private Island with a James Bond Backstory
Pitanguy Island is one of the most exclusive private islands in the world
Off the coast of Rio de Janeiro, in the Bay of Kings, sits a 75-acre island that has stayed almost entirely out of public view for half a century. Seen from the water, its white modernist bungalows curve along the shoreline, full-height glass catching the light, with a private airstrip cut into the hillside behind. The architecture is said to have been partly inspired by the lair of Francisco Scaramanga, the villain of The Man with the Golden Gun, who kept his own private island in the film. The real owner had no interest in world domination. but he understood the value of a place where the world's most recognisable faces could disappear completely.
From Big Pig's Island to a Private Brazilian Retreat
Before it had bungalows or an airstrip, the island was known locally as Ilha dos Porcos Grande, Big Pig's Island, for the shape of its silhouette. It was uninhabited forest and rock when Dr Ivo Pitanguy bought it in 1973.
By then, Pitanguy was one of the most famous surgeons in the world. His Rio clinic, founded a decade earlier, had become the destination of choice for royalty, presidents and Hollywood stars seeking cosmetic and reconstructive work away from the press. He wanted somewhere private to take his most high-profile patients to recover.
Working with Brazilian architect Paulo Coelho, Pitanguy reshaped the terrain of rolling hills, pathways, service roads, and the utilities needed to make the island liveable. The result was six modernist bungalows and nine suites in white render with curved walls and full-height glazing, alongside three private beaches, a natural aquarium, freshwater springs, a tennis court and a fleet of golf carts.

Celebrity Guests and a Surgeon Who Never Confirmed a Name
Pitanguy never spoke about who visited the island. Names still leaked into the press over the years; Tom Cruise and Katie Holmes, President Jimmy Carter, Jacqueline Kennedy Onassis, Brigitte Bardot, Queen Farah Pahlavi of Iran, Zsa Zsa Gabor among them. Though none were ever confirmed by the one person who would have known for certain.
Pitanguy's profile extended well beyond surgery. In the US, his skincare line was sold in Bergdorf Goodman and Neiman Marcus, at a time when South American luxury brands rarely reached either store. Less widely known: in 1960, over a decade before he bought the island, he opened a charity wing at Santa Casa Hospital in Rio, where he and his team carried out free reconstructive surgery for patients with birth defects or accident-related injuries, many of them children.
Pitanguy Island for Sale After Fifty Years
For more than four decades, the island stayed exactly what Pitanguy built it to be, a private retreat for family, close friends and a small circle of guests, never run as a business and rarely photographed.
That has now changed. For the first time in over fifty years, Pitanguy Island has come to market, listed for sale by the Pitanguy family at $100 million.

The island today has almost 15,000 sq ft of built space across its six bungalows and nine suites, set within 3.2 million sq ft of grounds, beaches and preserved forest, with the airstrip, helipad and marina pier still in place. Within existing planning consent, a buyer could refurbish or rebuild the current structures, in places extending upward to roughly 30,000 sq ft of two-storey villas. Subject to approval from the local municipal authority and the Brazilian Navy, there's also scope to build a single mansion of up to 30,000 sq ft at the island's eastern end, an addition estimated to add as much as $135 million to the island's value.
